Our goal is to use your time as efficiently as possible at PT Works. So we encourage you to download our patient information forms, complete them and turn them in when you arrive for your first visit. Please be sure to bring your prescription for treatment, all insurance information and a photo ID.

PT Works accepts many plans, including Aetna, Anthem (including Healthkeepers), Cigna (through Rehab Provider Network), First Health, Mamsi, Medicare, PHCS, Southern Health, Tricare and United Healthcare. We also accept Worker’s Comp.

Our staff will gladly contact your insurance company to verify your physical therapy benefits. We’ll also keep you informed as to any copayments, deductibles or visit limitations.
